The Case Manager provides comprehensive support services to chronically homeless individuals, including conducting assessments, developing service plans, and monitoring client progress. Stability Supportive Housing Program provides 60 units of scattered site supportive housing to chronically homeless single adults and adult families with a family member who has a serious mental illness and/or substance use disorder. The apartments are located throughout New York City.

Candidates must possess at least a high school diploma or GED with four years of relevant experience, or an associate/bachelor degree with corresponding experience. Proficiency in Microsoft Office and strong communication skills are required, with bilingual English/Spanish skills preferred.
