Case Management Coordinator Full-Time
The Case Management Coordinator gathers and evaluates patient data to identify treatment barriers and develop effective care plans. They facilitate communication between the patient, family, and healthcare team to enhance outcomes and promote patient empowerment.
Gathers all relevant data related to patient’s treatment status; objectively evaluates it to identifies barriers, clarifies realistic goals & seeks potential alternatives to care. Works with the patient/family, primary care physician & multi-disciplines to develop a treatment plan that enhances patient outcomes; facilitates communication & collaboration between the patient & health team; advocates patient empowerment in their own care.
Education: RN required, BSN preferred
Two to three years hospital experience required; Home Care and/or Utilization Review of Case Management experience desired.
Candidates must hold a valid RN license, with a BSN preferred. Two to three years of hospital experience is required, while home care or utilization review experience is desired.
