The practitioner provides direct in-person and telehealth primary care services across multiple FQHC sites to ensure continuity of care and support clinical operations. They collaborate with interdisciplinary teams to manage patient health, maintain accurate clinical documentation, and participate in quality improvement initiatives.
The Mid-Level Practitioner – Floater is a highly flexible clinical resource who provides direct patient care across designated FQHC sites in Southern Miami and the Florida Keys. The practitioner supports continuity of care, patient access, and clinical operations by providing coverage across multiple locations and programs based on organizational needs.
The Floater Practitioner will provide both in-person and telehealth care, including coverage for planned and unplanned provider absences, vacancies, recruitment gaps, fluctuations in patient volume, and other clinical staffing needs.
The successful candidate must be comfortable working across multiple clinical environments and demonstrate a high degree of flexibility, independence, initiative, adaptability, and responsiveness.
Candidates must hold a Master's degree in Nursing or Physician Assistant Studies and possess a current, unrestricted Florida license. A national certification, DEA registration, and BLS certification are required, along with the ability to travel between clinical locations.
