Case Manager

Minimum Qualifications

Bachelor's degree in social work, psychology, human services, behavioral health, or a related field preferred but not required; relevant case management or behavioral health experience may be considered in lieu of degree requirements. Experience working in behavioral health, human services, case management, or a related setting preferred. Strong communication, organization, and interpersonal skills. Ability to manage multiple client needs, referrals, appointments, and deadlines. Ability to work effectively as part of an interdisciplinary team. Comfortable working with individuals experiencing mental health and/or substance use disorders.

• PREFERRED EXPERIENCE Behavioral health or substance use treatment experience.
• Experience with PHP, IOP, outpatient, or residential services.
• Knowledge of community resources, Medicaid, benefits, housing, transportation, and other support services.
• Experience with discharge and aftercare planning.
• Familiarity with Motivational Interviewing and person-centered care.

ROLE SCOPE The Case Manager focuses on resource coordination, advocacy, social determinants of health, and continuity of care. Case Managers do not provide psychotherapy, diagnose clients, or independently perform clinical functions outside their professional credentials or scope of practice.
