To support the mission of Helio Health, the Continuing Care Coordinator will provide rehabilitation services to patients as a part of their overall treatment.
The Continuing Care Coordinator will work on a Full-Time basis out of Helio Health’s Rochester Inpatient Program.
Pay Rate: $23.00 - $27.00 per hour.
In addition to Helio Health’s comprehensive benefits package, Full-Time hires for this position are eligible for a Sign-On Incentive up to $1,500!
Maintain a case load of patients seeking treatment from the effects of substance use and/or mental health disorders. Perform essential counseling responsibilities including screening, intake and orientation, assessment, evaluation and intervention, treatment planning, referrals, crisis intervention, counseling, patient education, case management, reporting and record keeping. Participate in program, administrative and clinical committees as assigned. Relate effectively with staff, clients, and community while upholding confidentiality requirements.
Associate's Degree in a human service field with minimum of CASAC-T required. One year counseling experience preferred. Equivalent alternative combination of education and experience may be substituted for the educational requirements. Knowledge of addiction/recovery, behavioral health, self-help group process and principles of counseling. Proficiency with basic computer applications. Candidates must hold an Associate's degree in a human service field and possess a CASAC-T certification. One year of counseling experience is preferred, along with knowledge of addiction recovery principles.
