We are hiring an Admission Registered Nurse (RN) for a Hospice position providing end-of-life care to patients in their homes in the Lisle, IL. area. This position is eligible for a $9,000 first year success bonus to reward the accomplishment of key achievements during your first year!
Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
Conduct comprehensive hospice admission assessments, including clinical evaluations, medication reconciliation, and development of individualized plans of care aligned with physician orders
Educate and guide patients and families through the hospice admission process, setting expectations, explaining the hospice philosophy, and ensuring they feel informed, supported, and comfortable with the transition to end-of-life care
Apply strong clinical judgment and compassionate critical thinking to facilitate timely admissions, address immediate needs, and ensure accurate, thorough, and compliant EMR documentation
Initiate evaluations to other disciplines when indicated
Work independently, but never alone
Travel to patient homes within assigned territory
