St. Augustine Youth Services (SAYS) is seeking a Targeted Case Manager to join our COACHES Program. This position provides targeted case management services to children and adolescents with serious emotional and behavioral needs and works closely with youth, families, treatment teams, schools, and community providers. The Targeted Case Manager plays an important role in helping children and families access the services, resources, and supports they need to achieve their individualized goals. What You'll Do Serve as the Targeted Case Manager for approximately 1–3 children. Complete assessments and develop, update, and monitor individualized service plans in accordance with Florida Medicaid requirements.
  • Provide the five core functions of targeted case management: Planning Assessment Linking and advocacy Monitoring Discharge planning Work directly with children and families to address barriers and support implementation of their service plans. Connect children and families with appropriate community resources, providers, and services. Advocate for services and resources necessary to meet identified needs. Coordinate services with families, natural supports, treatment providers, schools, and other community partners. Participate in treatment planning and individualized service plan reviews. Attend required meetings, including Multidisciplinary Team (MDT), Integrated Practice Team (IPT), school, and community provider meetings. Monitor service delivery and document progress toward identified goals. Provide support and assistance during times of crisis, including helping families access appropriate resources. Coordinate discharge planning and aftercare/follow-up services. Participate in clinical care coordination and staffing with members of the child's treatment team, which may include physicians, psychiatric providers, therapists, teachers, attorneys, guardians ad litem, and other professionals. Maintain accurate client records, case notes, electronic health records, and other required documentation. Complete progress reports, service plan updates, and other required reports. Maintain knowledge of community resources and referral options. Assist with program data and reporting as needed. Participate in supervision, staff meetings, and treatment team meetings. Follow all SAYS policies, procedures, and applicable state and federal requirements. Perform other duties as assigned. Qualifications.

Requirements

Candidates must hold a bachelor's degree in a human services field and possess at least one year of experience working with children with serious emotional disturbances. Additionally, applicants must be eligible to obtain the Certified Behavioral Health Case Manager credential and maintain a valid driver's license.
