Overview

The Social Worker II provides a full range of social work services, including psychosocial assessment, treatment planning, therapeutic interventions, complex discharge planning, and crisis intervention for designated psychiatric patients. This role also assists patients and families in transitioning to the next appropriate level of care in collaboration with the healthcare team.
