They are responsible for following individual care plans, documenting services, and reporting any changes in a client's condition to the RN Supervisor.

The Nursing Assistant will deliver in-home care services following an individual Care Plan developed by the RN Supervisor, assisting with Activities of Daily Living (ADL) such as bathing, dressing, and eating, as well as performing light housekeeping and monitoring client health and safety. Responsibilities also include documenting all services provided, completing daily Aide records, and immediately communicating any changes in the client’s physical or mental condition to the RN Supervisor.
