The Home Health Registered Nurse administers skilled nursing care, performing various visits and assessing patients, while being responsible for the overall coordination of care delivered to their caseload. This involves regular patient visits, communicating with the Interdisciplinary Team and Medical Director, and implementing the plan of care based on physician orders.
Candidates must be graduates of an accredited nursing school and hold a current State License as a Registered Nurse, requiring a minimum of 1 year of experience as a nurse in a clinical care setting. Essential qualifications include reliable transportation, valid automobile insurance, and demonstrated skills in nursing practice and coping with ill or disabled patients.

Practice nursing with the autonomy, time, and patient connection that allows you to make a meaningful impact every day. As a Home Health Registered Nurse, you'll deliver comprehensive skilled nursing care to patients in their homes, supporting recovery, chronic disease management, and overall health goals while helping individuals remain safely in the comfort of their own environment.
This role is ideal for an RN who values clinical independence, critical thinking, and building strong relationships with patients and families over time. You'll manage a caseload of patients from admission through discharge, providing skilled nursing care, conducting assessments, and developing individualized care plans tailored to each patient’s needs. Working closely with physicians and an interdisciplinary care team, you'll coordinate services, make informed clinical decisions, and play a central role in shaping patient outcomes and supporting each person’s journey toward greater health and independence.
Pay Range $77,000—$88,000 USD
Employment is contingent upon the successful completion of a background check. Screening is completed by a third-party administrator, the Michigan Long-Term Care Partnership, and is performed in compliance with the Fair Credit Report Act.
