The Community Health Navigator provides care coordination and connects individuals with medical and behavioral health needs to social services and community resources. Key duties include conducting comprehensive assessments, developing and reviewing care plans, conducting risk assessments, and coordinating necessary referrals and service authorizations.
A High School Diploma or equivalent is required, though a Bachelor's degree in a Human Services field is strongly preferred, along with 1 to 2-3 years of case management experience, preferably with individuals experiencing Serious Mental Illness or Substance Abuse Disorder. Driving is a requirement, and candidates must be at least 21 years old and pass extensive background checks.
