Sparta Community Hospital is seeking a qualified Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to support both Hospitalist services and Rural Health Clinic (RHC) operations.
This position provides comprehensive patient care across the lifespan, while also supporting physicians through real-time clinical documentation (scribe responsibilities). The provider practices collaboratively with physicians and other healthcare professionals in a variety of care settings.
1. Clinical Care (RHC & Extended Care Settings)
Provide high-quality, patient-centered care in collaboration with a physician across multiple settings, including the Rural Health Clinic (RHC), Long-Term Care (LTC), hospital, and patient residences.
2. Scribe & Documentation Support
Provide real-time clinical documentation support to Hospitalist physicians to ensure accurate and efficient patient records.
