Candidates must be a Licensed Physical Therapist in New Mexico and possess at least one year of clinical experience, preferably in home health. Applicants should also be self-motivated, comfortable working independently, and proficient with electronic health records.

Key Responsibilities Evaluate patients and create personalized therapy plans in the home setting Deliver evidence-based interventions to restore mobility and function Educate patients and families on exercises and safety strategies Collaborate with the interdisciplinary team to ensure coordinated, goal-oriented care Maintain accurate and timely documentation to meet quality and compliance standards Adapt care plans based on progress and physician feedback Support patients through discharge planning and wellness strategies
