Key Responsibilities Evaluate speech, language, cognitive-communication, and swallowing disorders in home-based patients Develop individualized treatment plans based on physician orders and clinical assessments Provide direct speech therapy services to help patients regain or improve communication and swallowing abilities Educate patients and family members on exercises and strategies for ongoing progress Document patient progress, update care plans, and coordinate with interdisciplinary team members Participate in care conferences, in-service training, and performance improvement activities Maintain compliance with HIPAA and company policies for patient safety and confidentiality
