The Case Manager provides essential case management and housing support to assigned families, conducting needs assessments, home visits, and arranging necessary services while ensuring compliance with program parameters through monthly reporting.
Candidates must possess a Bachelor's degree in a relevant field, with a Master's degree preferred, and a minimum of two years of experience in Case Management, specifically working with homeless populations.

Under supervision of the Director of Social Service, the Case Manager is responsible for providing case management, housing support to clients and monthly reporting requirements that will ensure compliance with the parameters of the program. The requirements listed below represent the knowledge, skills, and/or ability required.
The salary for the Case Manager is $40,000 - $45,000/annually based on relevant experience.
