This part-time position offers a combination of outpatient services and PRN coverage in a skilled nursing facility (SNF) setting. Key Responsibilities Evaluate, develop, and implement individualized treatment plans based on physician referrals Provide skilled physical therapy services to patients in both outpatient and SNF settings Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes Maintain accurate and timely documentation in compliance with regulatory and company standards Collaborate with interdisciplinary teams to ensure coordinated patient care Supervise and provide direction to Physical Therapist Assistants (PTAs) and therapy aides, as applicable Educate patients and families on treatment plans, goals, and home exercise programs Why Join Trinity Rehab? Current North Carolina Physical Therapist (PT) license in good standing Graduate of an accredited Physical Therapy program Strong clinical skills and ability to work independently across multiple settings Excellent communication and organizational skills SNF experience preferred, but not required
