The Registered Nurse is responsible for providing patient training and ongoing support for home dialysis modalities, specifically managing the Peritoneal Dialysis program. This includes conducting home visits, assessing the patient's environment, and communicating patient status and interventions to the physician and team members.
Candidates must possess a current RN license and meet specific experience criteria: 12 months of clinical nursing experience and 6 months in the specific home dialysis modality for training. Additional requirements include CPR certification, successful completion of the Home Therapy Nurse Orientation program, and basic computer skills.
Description $10,000 SIGN ON BONUS AVAILABLE Southeastern Renal Dialysis is seeking an experienced Registered Nurse (RN) – Clinical Team Lead to oversee and coordinate our Home Dialysis Program for patients with End Stage Kidney Disease (ESKD).
This leadership role is responsible for delivering high-quality patient care while supervising and supporting RNs, LPNs, Patient Care Technicians, and Home Therapy Care Team Assistants. The Clinical Team Lead ensures clinical excellence, regulatory compliance, staff development, and strong patient outcomes.
Assess, plan, implement, and evaluate care for assigned home dialysis patients, providing ongoing clinical oversight Train and educate patients and care partners on safe, effective home dialysis therapy and treatment modalities, including transplant options Monitor treatment data, laboratory results, and patient response; adjust care plans and communicate updates to physicians and the interdisciplinary team Administer medications as prescribed and respond appropriately to clinical changes or emergencies Ensure accurate lab collection, processing, and documentation in compliance with regulatory standards Lead and oversee the home dialysis training program, ensuring high standards of patient competency and safety Supervise, mentor, and support clinical staff; assist with orientation and ongoing training Serve as a clinical resource and act as Nurse in Charge when assigned Ensure compliance with state, federal, and CMS regulations Support and drive quality improvement and infection control initiatives Participate in scheduling and rotating on-call coverage, including nights, weekends, and holidays as required
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Requirements include: Current RN license in applicable state. License must be maintained as current and in good standing. 12 months of clinical experience in providing nursing care and 6 months of experience in the specific modality for which the nurse will provide home dialysis training. Previous experience in providing nursing care to patients on maintenance dialysis preferred. CPR certification required prior to patient care assignment. Confirmation of ability to distinguish all primary colors. Must successfully complete the Home Therapy Nurse Orientation program and maintain annual demonstration of skills and competency applicable for the modality assigned. Demonstrated working knowledge of the English language and ability to communicate verbally and in writing. Must have basic computer skills; proficiency in all clinical applications required within 90 days of hire. Must meet any practice requirement(s) for the applicable state. Ability to meet minimum hiring standards which may include additional background clearances and orientation requirements if applicable to the program assigned to for employment.
