Responsible for assessment of health needs and contributing to the plan for care for individuals and families. Responsible for the initial implementation of nursing care plans, education of paraprofessional staff, and provision of nursing care as well as the periodic reassessment of individual and family nursing needs. Does related work as required. Must have availability of a motor vehicle. RESPONSIBILITIES Patient Care.
