This role serves as a Community Health Outreach Worker, connecting formerly incarcerated participants with essential health information and healthcare services while assisting in navigating those systems. The worker will also identify and help address social drivers of health such as housing, employment, and food barriers to successful societal re-engagement.
Candidates must have a history of incarceration with successful reentry at least three years post-release, and possess a valid North Carolina driver’s license with an acceptable driving record. A minimum of one year of experience in similar tasks within relevant service environments is required, along with good communication and basic computer skills.
The primary purpose of this position is to serve as a Community Health Outreach Worker for the Guilford County Formerly Incarcerated Transition Program. Works individually with participants to connect them with health information and health care services, while assisting in the navigation of these services. Identifies and assists participants in addressing the social drivers of health such as: housing, employment, food, and other barriers to successfully re-engaging in society after incarceration. This position works very closely with various community agencies and Health and Human Services organizations.
History of incarceration and successful reentry into the community after at least three-years from release. Requires a valid North Carolina driver’s license with a three-year acceptable driving record.
Requirements: & Experience:
Requires good written and oral communication skills, ability to work well independently and with a team and basic computer/word processing skills. Previous experience in peer support work desired. Some early morning, evening, and weekend work may be required. Demonstrated possession of knowledge, skills, and abilities as stated above through at least one year of experience in performing similar tasks in a hospital, clinic, nursing home, medical corps, prison or other social or rehabilitation services.
High school diploma or GED equivalent preferred. Previous experience in peer support work desired
