This role provides direct mental health case management services to adults, children, and families, involving collaborative care planning, referrals, and monitoring of treatment both within the agency and the community. Essential duties include connecting clients with resources, communicating with the treatment team, performing crisis assessments, and maintaining productivity and documentation standards.
A High School diploma or equivalent is required, though a Bachelor’s degree in a mental health related field is preferred, and experience in the mental health or substance abuse field is necessary. Candidates must possess an Ohio driver's license, proof of minimum $100,000 automobile liability insurance, and demonstrate strong organizational, communication, and computer skills.
Direct Clinical Service - Provides mental health case/care management services to adults, children and families; provides collaborative care planning, referral and monitoring of treatment. Coordinates services needed within the agency and within the community.
Ohio driver's license; proof of automobile liability insurance (minimum $100,000); Bachelor’s degree preferred in a mental health related field. High School diploma or equivalent required. LSW, LPC licensure not required. Experience in working in the mental health and/or substance abuse field.
