• Hours: Full-time (40 hours), Monday - Friday (8am-4pm)

Summary & Responsibilities

• The Day Program Case Manager is an integral role in our Day Program Services that will work to build and foster professional relationships with Individuals with developmental disabilities served on his or her caseload and members of each Individuals interdisciplinary team to ensure ongoing coordination of services.
