The Pharmacist will assess patient data to identify medication-related problems through reviews and rounding, and participate in medical emergencies like trauma, stroke, and sepsis alerts. They must review provider orders for appropriateness based on patient factors and consult with healthcare professionals regarding pharmacotherapy optimization.
A Pharmacy degree (Pharm.D.) from an ACPE approved college is required, along with either a successful completion of an ASHP-accredited PGY-1 Pharmacy Practice Residency or three years of hospital experience. Applicants must also be a registered pharmacist in Delaware.
