The Licensed Practical Nurse will provide skilled nursing services within the PACE clinic, focusing on supporting participant care transitions, including follow-up after hospitalizations, ER visits, and procedures to prevent readmissions. This role involves identifying high-risk patients, maximizing care coordination among all involved entities, and executing all components of the transitional care model.
Candidates must have completed a Licensed Practical Nurse program and possess one year of LPN experience, including proficiency in using electronic health records. An active Washington State LPN license is required, and experience with the geriatric population is preferred.

The Licensed Practical Nurse will provide skilled nursing services in the PACE clinic and support the transition of care for the PACE participants. This will include following up on patients post hospital discharge, post ER visits, post procedures, post-surgery and follow up on any acute changes or symptoms which may arise via patient phone calls and support the program goals to avoid readmission to hospitals and skilled nursing facilities. This position will help to manage all aspects of the care from inpatient stay to home. This role will: identify high-risk, complex patients and communicate with all entities involved in the care of the patient to promote and maximize care coordination; uses all components of the transitional care model and nursing process including assessment, triage, planning, implementing and evaluating care to meet the patients’ needs; and plans, organizes, and provides nursing services to clinic patients in conjunction with clinic Providers and Registered Nurses.
Education - Completion of Licensed Practical Nurse program. Experience - One (1) year of LPN experience is required, along with proficiency in utilizing electronic health records. Experience with the geriatric population is preferred.Other Requirement(s) - Active Washington State LPN license. Fluency in English and any API languages preferred.
