The Registered Nurse Case Manager will provide skilled visits addressing the full scope of a patient's needs in their home, focusing on helping them overcome real challenges and make meaningful progress in their daily life and recovery. This role involves building one-on-one connections with patients while utilizing clinical expertise to make independent decisions.
Candidates must possess a current license with the state Board of Nursing and have a minimum of one year of nursing experience, with home care experience being preferred. Applicants also need a valid driver's license, vehicle insurance, dependable transportation, and the ability to pass a background check, including performing tasks that may involve heavy lifting up to 50 lbs.
