About the Role

St Helens is seeking a dedicated Social Worker to join their dynamic Hospital Discharge Team. This role offers the opportunity to make a meaningful impact on patients' lives by ensuring smooth transitions from hospital to home.

What will your responsibilities be?

• In this role, you will carry out assessments and related tasks to facilitate timely hospital discharge for patients.
• You will support individuals to return home or transition to appropriate discharge settings with the necessary care arrangements in place.
• You will operate effectively in a fast-paced, high-pressure environment, ideally with prior experience in discharge planning.
• This is a full-time position (37 hours per week), involving a combination of office-based, ward-based, and remote working on a rota.
• You will collaborate closely with health and social care professionals to ensure safe, person-centred discharge outcomes.

Essential Requirements

• Social Work England registration
• Eligibility to work in the UK
• Full UK driving license
• Post-qualified experience
• Degree level or equivalent in Social Work
