Patient Care Coordinator

We are seeking a dedicated and organized Patient Care Coordinator to join our inpatient care team. In this vital role, you will serve as a central connection point between patients, families, treatment teams, and community resources, ensuring coordinated, compassionate care throughout the patient journey.

Key Responsibilities

• Manage a caseload of inpatient and/or residential clients Collaborate with interdisciplinary teams to develop, implement, and update treatment and crisis plans Act as a liaison between patients, families, schools, healthcare facilities, and community agencies Help families access mental health, educational, and community-based services Monitor delivery of care, track progress, and support discharge and aftercare planning Ensure all documentation, billing, and regulatory requirements are met Identify and address cultural, social, and systemic needs or barriers to care Participate in treatment meetings, rounds, and care coordination activities

Requirements

Required Qualifications: Bachelor’s degree in a human services field At least 1 year of full-time experience working with individuals with mental illness

Preferred Qualifications

2+ years of experience in case management or healthcare/social services Experience in an inpatient or residential care setting Familiarity with Medicaid, IEPs, mental health systems, and community resource navigation
