The Speech Therapist will evaluate patient needs to identify speech, language, cognitive, voice, and swallowing disorders, and then develop and implement individualized, evidence-based treatment plans in the home setting. Key duties include educating patients and caregivers, coordinating care with the healthcare team, and maintaining compliant clinical documentation.
Candidates must be graduates of an accredited Speech-Language Pathology program and hold a current SLP license in the state of practice, with the CCC-SLP preferred or required. A minimum of one year of Speech Therapy experience is required, with home health experience being preferred.
We are seeking a compassionate and motivated Home Health Speech Therapist (Speech-Language Pathologist – SLP) to join our team. This role provides skilled speech therapy services to patients in their homes, focusing on improving communication, cognition, voice, and swallowing to enhance safety, independence, and quality of life.
