The Occupational Therapist will evaluate patient needs, develop individualized plans of care, and deliver skilled occupational therapy services in the home setting to improve patient independence and quality of life. Key duties include addressing ADLs/IADLs, recommending adaptive equipment, coordinating care, and maintaining compliant clinical documentation.
Candidates must be graduates of an accredited Occupational Therapy program and hold a current state license for practice. A minimum of one year of Occupational Therapy experience is required, with home health experience being preferred.
