Tapestry Management Provider Full time Advanced Practice provider Job Description: Tapestry Management Providers (TMPs) play a crucial role in delivering comprehensive, in-person clinical support to healthcare facilities. They ensure proactive patient care through the review and discussion of Chronic Care Management (CCM), Remote Patient Monitoring (RPM), Behavioral Health Integration (BHI) & daily risk reports with facility healthcare professionals, including physicians, nurse practitioners, and social workers. THIS POSITION DOES REQUIRE TRAVEL IN ATLANTA AND THE SURROUNDING AREAS.
