The coordinator will pre-certify medications and home treatment therapies while ensuring patient safety and coverage, and will also perform delegated nursing functions under RN supervision. Responsibilities include collaborating with providers, accurately documenting care, providing patient teaching, and assisting with performance improvement activities.
Candidates must be licensed as an LPN in Ohio, maintain AHA BLS certification, and possess IV therapy certification, or alternatively hold a Master's in Social Work and be licensed as an Independent Social Worker in Ohio. Continuing education requirements must be met for license renewal.

• Licensed as an LPN in the state of Ohio; completes twenty four (24) contact hours of approved CE during each two (2) years licensed renewal period consistent with requirements of Ohio Boarding Of Nursing. Maintain continuous American Heart Association (AHA) BLS certification; certified IV therapy. Or Masters in social work, Licensed as an Independent Social Worker in the state of Ohio.
