The Care Coordinator will provide community care coordination using established service delivery models, facilitating team meetings and developing collaborative, strengths-based service plans for clients. Responsibilities also include conducting outreach, monitoring participant alerts for emergency room visits, and documenting all service plans and contacts in the electronic health record.
Candidates must possess a Bachelor's degree in a human services or related field along with at least two years of experience in the mental health field. Preferred qualifications include bilingual proficiency in Spanish and experience working with diverse child and family populations facing significant behavioral or social service needs.

We are now hiring for a full-time Care Coordinator at our Merrionette Park location.
The average starting salary for this position will fall in the range of $47,000 and $52,000 annually. Where candidates fall in this range will be based on skill and experience level.
• Driving for work required with transportation of clients preferred in a personally owned vehicle required.
• Satisfactorily pass all screenings and criminal background checks.
While performing the essential duties of this job, the employee is required to sit and stand for sustained periods of time, frequently talk, hear, use hands and fingers to feel, handle, and operate objects or controls, and occasionally stand, bend, stretch, stoop, kneel and crouch. The employee is required to have close visual acuity to perform an activity such as preparing and analyzing data and figures, transcribing, and viewing a computer terminal.
