About the role

The Licensed Practical Nurse renders nursing care based on individualized client needs, operating under the direction of a registered nurse. Key functions include preparing and administering medications, assisting with patient care, and communicating with physicians to develop care plans.

Position Title

Licensed Practical Nurse REPORTS TO: Director of Sites CLASSIFICATION: Non-Exempt CareSTL Health is seeking Licensed Practical Nurses (LPN) to join our team! The licensed practice nurse (LPN) is responsible for rendering nursing care in assigned area in terms of individualized client needs, according to dependent and independent nursing functions and conformance with recognized nursing techniques, procedures, and established standards based on the scope of practical nursing, under the direction/delegation of the registered nurse. ESSENTIAL FUNCTIONS: The following information is considered the definition of essential functions but does not restrict the tasks that may be assigned. The Licensed Practical Nurse may be reassigned duties and responsibilities at any time due to reasonable accommodation or other staffing reasons.

Communicates with physicians and develop working relationship and evaluates the patient to determine level of care, when the patient need further, home planning and other needs and action plans and reviews treatment goals and resources

Prepares and administers medications and treatments with knowledge of purpose and adverse effects of nursing care provided as well as prepares equipment and aids Physician during treatment and examination of patient while ensuring that patient tests are appropriate and necessary and maintains appropriate costs

Assists with providing patient care: records vital signs; discusses and records chief health concerns; performs tests; administers injections as well as administers prescribed medications and treatments in accordance with scope of practice

Completes expanded assessment of patient at time of initial/new clinic encounter , interprets medical documents and facilitates investigations, medical and treatment referrals and participates in the development and implementation of patient care policies and protocols

Helps patients understand their conditions and treatment options while providing education and literature about diagnosis while helping patient and family make informed decisions as well as explains procedures and treatments to patient to gain cooperation, understanding, and alleviate apprehension

  • Other duties
  • Places phone calls to pharmacies to order prescriptions; sets-up referral contacts
  • Refers cases where patients and/or family would benefit from counseling and coordinates the integration of social services/case management function
  • Evaluates patient satisfaction and quality of care provided and maintains awareness of comfort and safety needs of patient
  • Promotes effective and efficient utilization of clinical resources
  • Any additional assigned duties

Job requirements and qualifications

Education:

  • Graduate of an accredited school of Nursing required
  • Valid registration as a Licensed Practical Nurse in the State of Missouri required
  • Current AHA Healthcare Provider training (BCLS) required

Additional Information

Clinic hours are Monday - Friday 8 a.m. to 5 p.m.

11 paid holidays per year.
