The Case Manager is responsible for accurate and timely documentation, assessing client needs, and formulating strength-based Individual Service Plans (ISPs) in collaboration with the client and family. They will also provide crisis prevention/intervention, support, education, and coordinate access to essential community resources.
A minimum of a High School Diploma and three years of experience are required, though a Bachelor's degree in a related behavioral/mental health field is preferred. Candidates must be proficient with computers, complete mandatory training within 90 days, maintain reliable transportation, and obtain CPR/First Aid and NCI/CPI certifications.
Harbor is seeking a dynamic and dedicated Case Manager to join our Developmental and Behavioral Services team at our Port Sylvania location. This full-time position provides the opportunities to offer meaningful community-based support and resources to children and adolescents with a wide range of developmental and behavioral health needs. As a key member of our engaged and experienced interdisciplinary team, you will collaborate closely with therapists, psychiatric providers, and other professionals to ensure comprehensive, person-centered care.
Position is full time, 40 hours per week.
