The Case Manager III serves as the primary support and case manager for individuals with mental illness and co-occurring disorders, assisting them in achieving independent living within their chosen community. This role requires performing job functions proficiently, distinguished from lower-level Case Managers by additional education and experience.
Candidates must possess either a Bachelor's degree in Social Services or Human Services with one year of experience, or a High School/GED equivalent with four years of direct mental health or community services experience, with an Associate's degree substituting for two years of experience. A valid driver's license and personal vehicle insurance are mandatory for client transportation.

Under direct supervision, serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. Case Manager III is distinguished from Case Manager II by the additional education, years of experience, and ability to proficiently perform the job functions and meet position standards.
Expected starting wage range is $22.62 - $27.63. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

Based on the Assertive Community Treatment (ACT) model, STRIDES of Alameda County is an intensive community support service for 120 individuals with severe mental illness (SMI), between the ages of 18-62, who are considered to be at great risk for psychiatric hospitalization.
