Medical Case Managers coordinate care for clients diagnosed with HIV/AIDS, maintaining a caseload of approximately 50 active clients and acting as a liaison between clients and physicians. Responsibilities include developing standardized care plans, conducting regular counseling, identifying barriers to care, and making appropriate referrals.
Candidates must possess strong interpersonal, organizational, and communication skills, along with at least two years of experience in HIV/AIDS case management services. Education requirements mandate either a Bachelor's degree in social work or a related field, or an Associate's degree with intent to obtain a Bachelor's.

Medical Case Managers are responsible for the coordination of care for clients who are diagnosed with HIV/AIDS at the Care Clinic. The Medical Case Manager reports to the Health Center Administrator.
