The Physical Therapist performs all phases of physical therapy evaluation, treatment planning, treatment, and documentation in keeping with the Hospice/Home Healthcare philosophy. This includes administering modalities, maintaining accurate patient records, communicating with the medical team, and supervising Certified Home Health Aides.
Candidates must possess two years of professional experience in rehabilitation and hold a California Physical Therapist license, along with Basic Life Support (BLS-CPR) certification. Required abilities include effective oral and written communication, strong organizational skills, and the capacity to function collaboratively within a patient-care team.
