The Case Coordinator provides coordination services to patients in the Pathways Center, focusing on performing initial and annual assessments for HIV primary care clients and developing service plans to address identified needs. This role involves coordinating resources, providing supportive services like medication adherence education, and ensuring timely patient engagement in various care programs.
A Bachelor's degree is required, along with experience in HIV case management, treatment adherence, or care coordination, preferably with some supervisory capacity. Candidates must possess extensive knowledge of HIV transmission, prevention, adherence, and viral suppression, alongside strong organizational and communication skills.
Under the supervision of the Director of DAC Services and Programs, the Case Coordinator performs a variety of coordination services to all patients engaged in any clinical discipline within the Pathways Center. The Case Coordinator complies with the standards of care management for HIV+ primary care patients and assists other patients based on patient-centered identified needs.
