Conduct evaluations of residents’ mobility and design individualized physical therapy programs. Collaborate with interdisciplinary teams to ensure holistic care and maintain accurate records of assessments and treatments.
Candidates must be graduates of an accredited Physical Therapy program and hold a valid license to practice in Wisconsin. The position is open to both full-time and part-time applicants.
Competitive pay Sign-on bonus up to $6,000 for eligible full-time PTs only Health insurance options PTO and sick leave 401(k) with employer match Flexible schedules for full-time, part-time, and PRN roles Paid state licensure Supportive, therapist-led work environment Physical Therapist Responsibilities Conduct initial and ongoing evaluations of residents’ mobility, strength, balance, and endurance Identify barriers to functional independence and design individualized physical therapy programs Address challenges related to mobility, posture, gait, and injury or illness recovery Provide interventions to help residents regain strength, prevent falls, and maintain independence Support residents in daily functional tasks, recommending adaptive equipment as needed Conduct exercises and mobility programs to enhance coordination, flexibility, and overall physical function Assess living spaces to recommend modifications for safety and accessibility Educate residents, families, and staff on therapy goals, safe mobility, and fall prevention Collaborate with OTs, SLPs, nursing, and interdisciplinary teams to ensure holistic care Maintain accurate records of assessments, treatments, and progress, ensuring compliance with healthcare regulations and community policies Ready to Grow Your Career?
