First Choice Home Health & Hospice is seeking to hire PRN LCSW to provide support for Home Health and Hospice patients and families in Utah County. Work is performed directly in the homes of the patients (see below for more detailed duties). Flexible hours, and possibility of full time in the future as we continue to be a first choice in home health & hospice!
Job Requirements: LCSW
Assess patient’s socio-economic, psychological and emotional needs; Educate patients and family members regarding resources and act as liaison; Provide intervention in crisis situations; Performs spiritual assessment on all Hospice patients; Participating in interdisciplinary team meetings; Advocating on behalf of the patient and family; Assists patients and families in preparation of advanced directives; Assess the need of patient's and families for counseling related to the risk of pathological grief; Identify patient and family's needs when level of care changes or the patient is discharged
