The Clinical Staff Pharmacist for Transitions of Care - Transplant ensures patients are prepared for care transitions by reviewing medications for appropriateness, addressing discrepancies with prescribers, and providing comprehensive patient counseling on medication regimens. This role involves participation in specialty consults, high-risk patient discharge counseling, and identifying process improvements within the transitions of care program.
A Doctor of Pharmacy (PharmD) degree from an ACPE-accredited school is required upon hire, along with a Texas State Board of Pharmacy license and Basic Life Support certification. Preferred qualifications include completion of a PGY-1 residency and two or more years of clinical pharmacist experience.

Required Education Doctor of Pharmacy (PharmD) degree or equivalent from an American College of Pharmaceutical Education (ACPE) accredited school of pharmacy required. Experience Good general health and stress-coping ability. Licenses and Certifications (PHARMD) DOCTOR OF PHARMACY Upon Hire Licensed Pharmacist by Texas State Board of Pharmacy (TSBP) Upon Hire (BLS) BASIC LIFE SUPPORT certification with a course accredited by the American Heart Association (AHA) Upon Hire Preferred Experience Completion of a PGY-1 Pharmacy Practice Residency from an ASHP-accredited residency program preferred. 2 years or more of clinical pharmacist experience
