Salary Range
$52,000 - $60,000 per year

Essential
Duties and Responsibilities
Conducts screenings and level of care assessments, using clinical knowledge and skill level and communicate the process with clients.
Evaluates behavioral, emotional, cognitive, spiritual, and social needs of clients
Collect demographic and historical information relevant to the LOCA
Gather information for and complete precertification for all managed care clients when applicable.
Facilitate admission process by communicating with appropriate clinical team member as required.
Facilitates appropriate admissions to all levels of care
Makes appropriate referrals to community providers and finds placement in appropriate setting for the client.

Completes assessment, admission, and other paperwork for clients to in a timely manner. Including but not limited to Releases of Information (ROI) and other admission paperwork
Manage admission pre-certification with funding sources when applicable.
Collaborates with other team member, and departments as needed.
Expected to provide coverage for other clinical team members
Responsible for maintaining clinical record for clients with appropriate documentation of clinical services and client program engagement in ECR in accordance with program policy.
Conducts brief individual therapy to engage patient in treatment and documents appropriately.
Provides case management and linkage to alternate services if indicated.
Develops an initial treatment plan.
Completion of all required trainings as designated by the company and accreditation/licensing entities.
Expected to provide and facilitate group therapy sessions as required.
Other duties as assigned.
