The Care Manager will provide comprehensive care management services to HARP and Health Home Plus participants, including conducting assessments and home visits. They will collaborate with interdisciplinary teams to develop and update care plans while ensuring maximal access to medical and social services.
Candidates must hold a bachelor's degree in a relevant field or possess specific professional credentials such as a CASAC or RN license. The role requires experience in providing direct services to individuals with behavioral health or developmental disabilities.
Under the supervision of the Clinical Supervisor of Health Home Plus and HARP, the HARP/Health Home Plus Care Manager will provide care management services to program participants who are in need of assistance with medical, behavioral health, and/or social services in the community.
