About the role

The clinician is responsible for completing assessments and developing an Individualized Rehabilitation Plan (IRP) for individuals with serious mental illness in the community. Additionally, the role involves providing clinical oversight to case managers to support consumers in maintaining independent housing.

SHARE (Supportive Housing Assistance to Reach Excellence) is a supporting housing service designed to assist persons with serious mental illness(es) in maintaining independent housing in the community.

The case managers provide the support needed in each person's home environment and in the community.

SHARE provides outreach and case management services to program participants.

The clinician works with these individuals in the community to complete assessments and develop an Individualized Rehabilitation Plan (IRP).

Additionally, the clinician provides clinical oversight for the case managers to support consumers.

Responsibilities

Perform therapeutic rehabilitative skill development Maintain accurate documentation and client records Recommend referral services as needed Develop, implement and maintain client IRP Provide education to client systems related to nutrition, symptom management, medication safety and management, and discharge planning Provide transportation to clients as needed Provide crisis intervention services as needed Graduation from an accredited college or university in Social Work, rehabilitation counseling, psychology, criminal justice, counseling, or other related behavioral healthcare or counseling LAC/LSW required One year of experience in the psychiatric/mental health field Valid N.J. drivers license with less than 6 points Full Benefits 12 Paid Holidays Sick Days Personal Days Accrued Vacation Medical/Dental/Vision Company paid Life Insurance and Long-Term Disability 403B Plan with Company Match Opportunities for training/education/Continuing Education Credits Opportunities for Public Loan Forgiveness Opportunities for Tuition Discounts from Participating Institutions

Requirements

Candidates must have graduated from an accredited college or university in a relevant field such as Social Work, psychology, or counseling, and must possess a valid LAC/LSW license, with LPC/LCSW being preferred. A minimum of one year of experience in the psychiatric/mental health field and a valid N.J. driver's license with fewer than 6 points are also required.
