Position Overview We are seeking a compassionate and skilled LVN to provide direct patient care.
This role is essential in supporting hospice patients and families with symptom management, comfort care, and timely interventions during critical moments.
Key Responsibilities Provide direct patient care under the supervision of a Registered Nurse, following the established plan of care.
Respond to on-call needs including symptom management, medication administration, and emotional support.
Accurately document all patient interactions and care provided during the shift.
Educate patients and caregivers on care techniques and symptom management.
Communicate effectively with the interdisciplinary team regarding patient status and needs.
Ensure appropriate use of equipment and supplies.
Evaluate patient response to treatments and medications.
Participate in quality improvement activities and professional development.
Perform other duties as assigned by the supervising RN.
Qualifications Graduate of an accredited vocational nursing program.
Active LVN license in the State of California.
Minimum 3 years of nursing experience; hospice or community health experience preferred.
Current CPR certification.
Valid driver’s license, reliable transportation, and auto insurance.
Strong verbal and written communication skills.
Ability to work independently during overnight hours.
Understanding of hospice philosophy and the needs of the terminally ill.
