This RN is responsible for providing skilled professional nursing care, coordinating the interdisciplinary care team, and managing patient needs across various age groups, including triaging after-hours calls and responding with necessary visits. Essential functions involve completing patient assessments, revising care plans, documenting thoroughly, promoting patient comfort, and ensuring compliance with physician orders and regulatory requirements.
Candidates must be currently licensed as a Registered Nurse in Maryland, possess current CPR certification, and have at least two years of general nursing experience, preferably with Hospice, medical, surgical, critical care, or home health background. A Bachelor of Science in Nursing is required, or an Associate degree with an agreement to obtain a BSN within five years, alongside a reliable, insured vehicle.
30p, rotating holidays and occasional call. This RN is responsible for triaging after-hours calls and responding with a skilled nurse visit as appropriate. Will provide skilled professional nursing care to the patient as prescribed by the physician and in compliance with regulations as established by the State Board of Nursing and department policies. The RN/Case Manager coordinates the interdisciplinary care team for the patient and family to assure continuity of care. Services may be provided to neonate, infant, child, adolescent, adult and geriatric populations.

Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient’s status. Possess the ability to interpret the appropriate information needed to identify each patient’s requirements relative to his/her age-specific needs and to provide the care needed as described in the unit’s/department’s policies and procedures.
Medium-Heavy Work – Lifting up to 75 pounds on an infrequent basis (less than one lift every three minutes) and/or carrying up to 35 pounds while walking. Pushing or pulling up to 115 pounds infrequently or greater than 115 pounds occasionally (no more than once per hour).
