Duties and Responsibilities

• Provide face-to-face substance abuse psychosocial assessments
• Daily review and update of completing assessments, and other duties assigned by the program director
• Facilitate patient referrals for additional services consistent with assessed needs and physician recommendations.
• Meet patients for follow-up reviews or updates as needed or required
• Provide brief interventions, stabilization services and supportive counseling to patients
• Under the direction of the program director, the care coordinator will provide patients with medication education print-outs that are focused on understanding illness, symptom management and medication compliance
• Complete and file documentation as required in a timely manner

• Checks hospital email daily.

• Performs other related duties as assigned.
