The Nurse Case Manager will conduct medical assessments, develop and monitor healthcare plans for children in foster care, and provide direct medical treatment within the scope of nursing practice. Key duties also involve facilitating communication between all stakeholders and ensuring timely medical screenings and follow-ups.
Candidates must be a Licensed Registered Nurse (RN) in New York State, preferably with a BSN, and possess a minimum of two years of nursing experience, ideally in pediatrics or community health. A valid NYS driver's license and strong skills in care coordination, documentation, and EMR proficiency are mandatory.
Child and Family Services (C+FS) Foster Care Program provides temporary, safe, and loving homes for children and supports parents/caregivers in developing the skills they need to achieve family reunification. When reunification is not possible, our program helps secure permanency through adoption. This program plays an essential role in our agency’s 150+ year mission of nurturing a healthier and more equitable Western New York.

Competitive Pay Range of $34-$36 per hour based on experience
