The Family Medicine Physician will provide comprehensive, continuous medical care across the lifespan, focusing on preventive care, diagnosis, and treatment of acute and chronic conditions. Key duties include performing patient histories, physical examinations, developing treatment plans, and coordinating care with specialists.
Candidates must possess an MD or DO from an accredited medical school and complete an accredited Family Medicine residency program. Eligibility for unrestricted medical licensure in Illinois and DEA registration are also mandatory qualifications.
Family Medicine PhysicianPosition Summary The Family Medicine Physician provides comprehensive, continuous medical care to patients across the lifespan. This role focuses on preventive care, diagnosis and treatment of acute and chronic conditions, and long-term patient relationships while coordinating care across the healthcare continuum. Key Responsibilities Provide primary care services to pediatric, adult, and geriatric patients Perform patient histories, physical examinations, and clinical assessments Diagnose and manage acute illnesses and chronic medical conditions Deliver preventive care, including health screenings and immunizations Develop and implement individualized, evidence-based treatment plans Order, interpret, and document diagnostic tests and results Perform office-based procedures within scope of practice Coordinate care with specialists and ancillary services as needed Educate patients and families on disease prevention, wellness, and self-management Maintain accurate, timely, and compliant medical documentation Participate in quality improvement and patient safety initiatives Practice Setting Outpatient primary care clinic with optional inpatient or hospital rounding, depending on practice model May include call coverage or extended hours as required Patient Population Patients of all ages, from newborns to geriatrics Broad range of acute and chronic medical conditions
