Salary
$120,000 - $143,000 / YEAR
Providers will deliver comprehensive care to patients in their homes or community facilities, conducting frequent in-home visits for assessments, exams, and medication reviews while managing chronic conditions. They will also serve as the primary care provider, educate patients, and collaborate with interdisciplinary teams to ensure coordinated care.
Candidates must possess an active, unrestricted license as a Nurse Practitioner or Physician Assistant in the state of practice, along with relevant certification and the ability to obtain autonomous provider status within six months. Required qualifications also include a clean driving record, DEA license, BLS certification, and the ability to pass extensive background checks.

As a Provider at Lucet, you’ll deliver comprehensive care to patients in their homes and/or community-based services such as assisted living or skilled nursing facilities, helping to improve outcomes and promote continuity of care.
