The provider will conduct in-home visits to perform assessments, manage chronic conditions, and develop individualized treatment plans for patients. They will also collaborate with interdisciplinary teams and maintain accurate EMR documentation to ensure coordinated, high-quality care.
Candidates must hold an active, unrestricted license as a Nurse Practitioner or Physician Assistant with certification in Family or Adult-Gerontology practice. A minimum of two years of experience in primary or longitudinal care is preferred, along with a clean driving record and the ability to pass a comprehensive background check.

As a Provider at Lucet, you’ll deliver comprehensive care to patients in their homes and/or community-based services such as assisted living or skilled nursing facilities, helping to improve outcomes and promote continuity of care.
